Application Instructions

Application: Once you begin the application, you may save it and work on it later. Once you have completed your application, click on “Submit Form” in the upper right hand corner of the PDF document to submit it electronically. (I can’t find the “Submit Form” button. Where do I click?)

Reference Letters: Two letters of reference are required following your online application submission. Please use the form provided below for each letter of reference.

• Letter #1: One letter is to come from your supervisor. If you are self-employed, the letter should come from someone in the community.
• Letter #2: One letter is to come from a colleague or service organization in which you are involved.

Email letters of reference to cweber@fmwfchamber.com. Deadline to apply: Completed online application and emailed letters of recommendation are due by Friday, May 13, 2011 at 4 p.m.
